Since every practice and every scenario are different, a hybrid approach involving various communication methods can be used, said Dr. Jonathan Berlin of NorthShore University HealthSystem. "The most important thing is to have an awareness of the problem so that we can address the problem," he said. "Communications tend to evolve over time."

In critical results reporting, the first step is one-way communication, such as calling, sending an email, and sending a fax, and "ensuring that the report was received," Berlin said. "And of course that needs to be documented."

Many healthcare providers and entities use an EMR system, and some of these systems have email programs to assist in communication, he said. An email receipt can document communication, for example.

"Communication is facilitated with an EMR, but it's not necessarily enough," he said.

Commercial products also are available to assist in this task, and options may include email or text messaging, according to Berlin. Ordering physicians can often specify which mode of communication they prefer, and these tools can be integrated into the EMR.

Electronic communication issues

For all its benefits, electronic communication also has issues to be aware of, including physician demographics. By 2020, half of U.S. physicians will be 65 or older by 2020, and many physicians in this demographic may not be as comfortable with some electronic communication methods, Berlin said.

"We have to be cognizant of the fact that in different demographics, different people prefer different communication [methods]," he said.

In addition, subtleties are often not conveyed during electronic communication, Berlin noted. It also does not facilitate confirmation of the understanding of the conveyed information.

"Electronic communication, while it's very important, there also has to be some element to supplement different kinds of communication and voice communication," he said.

Closed-loop communication, which attempts to address the problem of incomplete understanding of conveyed information by having respondents "read back" what they think they heard to confirm understanding, would seem to be an ideal method. However, this technique can be difficult to use as medical decision-making can have many subtleties and nuances, Berlin said.

"Sometimes it doesn't fit, but it's certainly something that we have to try to achieve," he said.

In fact, the Joint Commission's No. 2 goal in improving communication recommends that critical test results be read back to the person who first delivered the test results.

Communication approaches evolve over time, and new methods may potentially facilitate handling critical results communication, Berlin noted.

"As long as these companies believe that it is okay to sell this technology to dictators, democracy activists, human rights activists, bloggers, and journalists around the world will continue to suffer," the EFF writes. By calling out these companies, the advocacy continues to urge the United States and European Union to adopt "know your customer" standards, which would prevent Western companies in selling to governments known for violating human rights.?

During the height of Syria's violent crackdown on democratic protesters in March 2011, Area SpA employees were flown into Damascus, the country's capitol, to finish a project that would allow the government ?to intercept, scan and catalog virtually every email that flows through the country,? Bloomberg reported.

Following protests outside of Area SpA's Italian office, the company announced it would no longer honor the contract with the Syrian government, stating that it was Area SpA was ?against all forms of repression and disapproves of any use of technology for violating human rights.? The equipment used in the government's escalating crackdown was already in place however. And as the EFF notes, y the time "Area SpA claimed it would exit the country in November, the civilian death toll in Syria already stood at more than 3,000."

The EFF describes Germany-based Trovicor as "perhaps the most prolific of the mass surveillance companies, having sold spy technology to a dozen countries in the Middle East and North Africa."?

The former Nokia-Siemens subsidiary counts the governments of Iran, Bahrain and Tunisia as its customers, three countries active in the Arab Spring uprisings. ?

Nokia Siemens divested from Trovicor after it was revealed the company sold spy tech to the Iranian government following the post-election uprisings in 2009.?

In Bahrain, Trovicor still maintains the monitoring centers it originally installed that helped surveil emails, text messages and phone calls. "Almost two-dozen former political prisoners recently testified to the England and Wales lawyers association that they were beaten and subsequently interrogated while being shown transcripts of emails and text messages," the EFF notes. "There have been at least 140 documented allegations of torture in Bahrain in the past last year."

Trovicor is also among the companies that sold spy tech to former president Ben Ali, and the EFF calls out Bloomberg's report on Trovicor?s dangerous capabilities:

[Trovicor?s] toolbox allows more than the interception of phone calls, e-mails, text messages and Voice Over Internet Protocol calls such as those made using Skype. Some products can also secretly activate laptop webcams or microphones on mobile devices. They can change the contents of written communications in mid-transmission, use voice recognition to scan phone networks, and pinpoint people?s locations through their mobile phones. The monitoring systems can scan communications for key words or recognize voices and then feed the data and recordings to operators at government agencies.

In the coming weeks, the EFF will continue to profile the dozens of companies in the U.S. and the European Union that supply equipment to countries known for human rights violations, promising to continue until "Congress and the EU countries act to prevent more of this dangerous technology from falling into the wrong hands."

At this point, what usually happens is that he just wants to get the thing sold as easily and quickly as possible. Sure looks that way according to a new T-Mobile accessory document that has turned up. We've heard quite a bit about the HTC Ville device itself -- even caught some images and video of it in action but until now there has been no mention of possible carriers for the device. Also noted in the document is the Huawei Prism and the Samsung T769 though, overall the information is lacking aside from the names being called to the front. Either way, it'll be interesting to see where the HTC Ville ends up given its rise to fame thus far.

Personal Best: Workouts May Not Be the Best Time for a Snack

A few weeks ago, a friend showed up for a run with a CamelBak ? one of those humplike backpacks with a tube that allows you to sip liquid ? and a belt containing food to eat along the way. Every 20 minutes or so as we ran, he stopped to eat and drink, sprinting afterward to catch up.

Now that is unusual, I thought. Does it really help to eat so often during a 16-mile run?

Certainly a lot of athletes believe they need constant nourishment. My friend and running partner Jen Davis, who has entered more races and run more than I ever have, once went on a 30-mile training run with a guy wearing a CamelBak and bearing snacks. He stopped every 20 minutes along the way and then, about halfway through the run, pulled out a turkey sandwich.

?I?m not sure if he ever actually ran an ultra race,? Jen said. ?He may have gotten injured after carrying that heavy pack on those long runs.?

There is no end to the crazy foods people will eat at endurance events. At the J.F.K. 50-Mile in Maryland, boiled potatoes and chicken broth are provided at aid stations. At the Rocky Raccoon Endurance Trail Run in Texas, runners can choose rice and beans or pasta, along with snacks like pretzels, cookies and candy.

At a 100-mile bike ride my husband and I have done several times, pumpkin pie is offered about 25 miles from the finish line. (My husband tried it one year and felt ill the rest of the ride.)

For the athlete determined to munch on the go, there are shelves worth of prepackaged ?energy gels? and bars, even jelly beans, promising to raise performance.

But most athletes are not running 30 or 50 or 100 miles, nor are they doing the equivalent amount of exercise in another sport, like cycling or swimming or skiing. So most of us really do not need to keep eating during a race to maintain energy and stamina, said Nancy Rodriguez, a sports nutritionist at the University of Connecticut.

Dr. Rodriguez reviewed published studies on nutrition and performance as part of a group of experts who wrote a position paper on the topic for the American College of Sports Medicine. Runners, for example, competing in a 5- or 10-kilometer race, she said, ?don?t need the CamelBaks and don?t need to have that Hershey bar or Powerade or Clif shot.?

Even athletes who are fast and competitive may not always need to eat during a workout. There?s no set rule on what they should eat and drink before, during and after exercise, said Melinda M. Manore, a sports nutritionist at Oregon State University who was an author of the position paper.

?People have gotten the message that they have to eat something,? Dr. Manore said. They guzzle an energy drink or eat a sports bar, but that doesn?t help. And for the many who are trying to lose weight, the habit just adds extra calories.

What they need depends on what they ate before they started and how hard their workout is going to be, among other things, she explained. ?If you can run six-minute miles or five-minute miles and you are going out for an hour, you do not need to be eating an energy bar during the workout,? Dr. Manore said.

Moderate athletes need to eat and drink after the workout, she said, but a healthy meal with plenty of fluids is sufficient. Indeed, for most of them, the most common error is to eat too much.

Dr. Manore follows her own advice. She hikes for an hour in the hills every morning, four to five miles. All she has before she goes out is a cup of tea with milk.

But anyone exercising for two hours or more does need to get carbohydrates, the muscles? fuel, according to the position statement. That means eating before, and perhaps during, the workout.

Those who try to skimp can end up with a poorer performance, said Dan Benardot, a sports nutrition researcher at Georgia State University. A long workout, like a run that lasts more than two hours, is ?an enormous drain on blood sugar,? he said.

If the body runs out of glucose for fuel, it will start breaking down muscle, which is counterproductive. Dr. Benardot?s research indicates that athletes do best when they never let themselves have more than a 400-calorie deficit during the day. That is, if you expend 1,500 calories on a two-hour run, you offset it with at least 1,100 calories in food that day.

That means it is a disadvantage to eat most of the day?s calories at one time ? at night, for example. But athletes should make dietary changes gradually so their bodies can adapt to more frequent fueling, he said. Those who try sudden changes sometimes pay a price.

Dr. Benardot tells the story of a distance runner who was doing well and felt great the morning of a big marathon. Before the race began, she saw her chief competitor put packs of a sugary gel into her running bra to eat during the race.

The distance runner did the same, even though she had never before eaten during races or long runs. It was a disaster: She had diarrhea during the event.

The gels ?were anything but a competitive advantage,? Dr. Benardot said.

?You have to let your body adapt,? he added. ?And you have to find out what works for you.?

Vancouver, B.C. (Wednesday, February 15, 2012) In 1997, a forest fire in Indonesia ignited an area of peatlands that smouldered for months. By the time it was over, the fire had released greenhouse gases equal to 20 to 40 percent of the total worldwide emissions that year from fossil fuels.

But that could be a drop in the bucket compared to future emissions from peat fires. Indonesian peatlands are dwarfed by Canada's. The total area of all peatland in Canada is estimated to be about twice the size of Saskatchewan.

At this week's meeting of the American Association for the Advancement of Science (AAAS) in Vancouver, Douglas Woolford of Wilfrid Laurier University will present findings that show how the fire season is becoming longer, and Mike Flannigan of the University of Alberta will highlight the increased risk of peat fires.

During a forest fire, especially in years of drought, peat can also ignite. When that happens, it produces a smoldering, smoky burn that is difficult to extinguish. Peat can grow several meters deep beneath the ground. In fact, some peat fires burn right through winter, beneath the snow, then pick up again in the spring.

A warming climate appears to be increasing the risk of peat fires in the North, according to Flannigan. For example, in 2007, Alaska's Anaktuvuk River region experienced a "tundra fire" fuelled by peat that covered 1,000 square kilometres. Until then, fire had largely been absent from the tundra since the Holocene epoch12,000 years ago.

Woolford's statistical analysis has shown that the forest fire season is becoming longer. Now, climate change models have been used to predict greater risk of forest fires in the future. On top of that, a warming climate means dryer weather, which makes peatlandsrich in legacy carbonmore likely to ignite and release greenhouse gas emissions. This would further contribute to global warming, creating a vicious circle of environmental harm.

Woolford and Flannigan will present their findings at the AAAS session Forest Fires in Canada: Impacts of Climate Change and Fire Smoke that will take place on Sunday, February 19, from 8:30 to 11:30 a.m., in Room 109 of the Vancouver Convention Centre West Building.

The session will be the subject of a special AAAS news briefing for media in Vancouver on Saturday, February 18 at 2:00 p.m. (PST).
